Background of the Study

Obstetric fistula, a severe and debilitating condition that results from prolonged labor without adequate medical intervention, remains a major health issue in many low-resource settings, particularly in Sub-Saharan Africa. It occurs when a hole develops between the vagina and the bladder or rectum, leading to incontinence and severe social stigma. Nurses, as essential members of the healthcare team, play a critical role in the prevention, early detection, and management of obstetric fistula (United Nations Population Fund [UNFPA], 2023).

State Specialist Hospital, Bauchi, a key healthcare facility in northern Nigeria, often treats women who have suffered from obstetric fistula. While the condition can be prevented with timely access to skilled obstetric care, many women in rural areas still face significant barriers to receiving appropriate care during childbirth, such as insufficient healthcare infrastructure, limited access to skilled birth attendants, and delayed referrals. Nurses have the potential to mitigate these risks by promoting antenatal care, educating women about the importance of early and regular checkups, and identifying risk factors that may lead to fistula development (Abubakar & Aliyu, 2024).

This study aims to evaluate the role of nurses in preventing obstetric fistula at State Specialist Hospital, Bauchi, focusing on their involvement in educating women, providing early interventions, and improving healthcare practices to reduce the incidence of fistula.

Statement of the Problem

Despite increased awareness and efforts to reduce obstetric fistula, the condition remains a significant challenge at State Specialist Hospital, Bauchi, due to factors such as delayed access to skilled care, insufficient antenatal education, and lack of adequate infrastructure. Nurses, being at the forefront of patient care, have a crucial role in addressing these issues, but their current involvement in fistula prevention is under-explored. This study seeks to examine the specific roles nurses play in preventing obstetric fistula and the challenges they face in the process.

Objectives of the Study

1. To examine the role of nurses in preventing obstetric fistula at State Specialist Hospital, Bauchi.

2. To identify the challenges nurses face in preventing obstetric fistula.

3. To recommend strategies for enhancing nurses' involvement in the prevention of obstetric fistula.

Research Questions

1. What role do nurses play in preventing obstetric fistula at State Specialist Hospital, Bauchi?

2. What challenges do nurses face in preventing obstetric fistula?

3. What strategies can be implemented to improve nurses’ contributions to obstetric fistula prevention?

Research Hypotheses

1. Nurses play a significant role in preventing obstetric fistula through education, early detection, and promoting skilled birth attendance.

2. Lack of training, inadequate resources, and cultural factors hinder nurses' ability to prevent obstetric fistula effectively.

3. Increasing nurse training and community outreach programs will significantly reduce the incidence of obstetric fistula at State Specialist Hospital, Bauchi.

Scope and Limitations of the Study

This study will focus on nurses working in maternal health and obstetrics at State Specialist Hospital, Bauchi. Data will be collected through interviews, surveys, and a review of patient records related to obstetric fistula cases. Limitations may include variability in nurses' levels of expertise and the absence of systematic data collection on fistula prevention at the hospital.

• Obstetric fistula: A severe birth injury that causes a hole between the vagina and the bladder or rectum, usually due to prolonged labor without adequate medical intervention.

• Skilled birth attendance: The presence of a trained healthcare professional, such as a nurse or midwife, during childbirth to manage complications and provide care.

• Antenatal care: Medical care provided to a pregnant woman before childbirth, aimed at preventing complications and ensuring a healthy pregnancy.
